Client’s questionnaire: filing for divorce in Ukrainian courts

For a lawyer in in Kyiv/Ukraine to draft all the required documents and file for divorce in a Ukrainian court, the client will need to provide the information and scans of documents specified below:

Scan of the husband’s passport;
Scan of the wife’s passport;
(If applicable) Scan of the husband’s individual tax number;
(If applicable) Scan of the wife’s individual tax number;
Scan of the marriage certificate;
(If applicable) Scan of the birth certificate/certificates of the child/children the spouses had together;
The husband’s email address;
The wife’s email address;
The husband’s phone number;
The wife’s phone number;
The husband’s registered place of residence (with a postal code);
The wife’s registered place of residence (with a postal code);
Do both of the spouses consent to the divorce?
When did the marital relationship end?
Is there a dispute regarding the children?
Is there a dispute regarding the spouses’ marital property?
Is there a dispute regarding alimony (maintenance payments)?
Would it be necessary to affix an Apostille to the court decision on divorce? If so, for what country?

On the basis of the aforementioned documents, the lawyer will be able to provide legal advice regarding the matter of divorce with a Ukrainian citizen, prepare the divorce petition and present the case in court.
