Questionnaire for shareholder: LLC changes

In order for a lawyer to make changes to information about Ukrainian LLC in the USR, the following data is required:

2. Scan of the charter of LLC (or “Inventory”)
3. The changes

   4. Shareholders’ data
   5. Change in the amount of the share capital
   6. Codes for the type of economic activity
   7. Contact details (email, phone)
   8. Transition to the model charter
   9. Constituent documents

4. Number of the previous minutes (decisions) of the meeting of participants
